If you’re looking to screen guests before your tours or activities – or in fact within any industry – you might be looking for a clear template that can help to identify common COVID-19 risk factors. These questions could be added into your liability waiver form, or provided as a separate form. As guests arrive onsite, you could either direct them to scan a QR code that links to your COVID-19 questionnaire, or to a check-in kiosk with your custom Wherewolf app installed.
What should be included in a COVID-19 questionnaire?
While your own questionnaire should be tailored according to your location, industry and local regulations, here are the essentials of what you might include.
- First name
- Last Name
- Phone number
- Email address
- In the past 24 hours have you experienced:
New loss of taste or smell
Head or muscle aches
Runny or stuffy nose
Shortness of breath
- Have you recently been in close contact with anyone who has exhibited these symptoms?
- Have you been in close contact with anyone who has been in quarantine or diagnosed with COVID-19 within the past 14 days?
- Have you recently travelled from an area with community spread of COVID-19?
If a guest answers yes to any of the risk factors, you’ll be able to spot these responses in your guest manifest on the day and respond appropriately. It can be worthwhile having a refund or rebooking policy in place, so guests feel they can be honest with their responses without the fear of losing their booking.
How do I set this up within a Wherewolf electronic waiver?
There are a few different ways you can ask about COVID-19 risk factors in Wherewolf. We’ve outlined some of the possibilities over at our blog post What COVID Questions Should You Ask Guests? including using checklist and multi-select options, or including requirements in your terms and conditions.
What about a liability waiver template?
Because each activity, tour and experience is unique, we advise you to speak with your legal advisor to develop the right liability waiver template to suit your business and potential risks. The team at Wherewolf is happy to help you set up your custom waiver form to ensure no detail will be missed. If you integrate a reservation platform, the system can automatically send a waiver form to each guest to complete a set time ahead of arrival. Any guests who haven’t completed their waiver by the time they arrive can do so via your QR code or web link.
In 2020 we decided to make our base plan free for up to 400 monthly waivers to support tour and activity operators just like you. You can get started for free, and try Wherewolf Lite for yourself now.
