A Sample COVID-19 Questionnaire for Your Tours or Activities

If you’re looking to screen guests before your tours or activities – or in fact within any industry – you might be looking for a clear template that can help to identify common COVID-19 risk factors. These questions could be added into your liability waiver form, or provided as a separate form. As guests arrive onsite, you could either direct them to scan a QR code that links to your COVID-19 questionnaire, or to a check-in kiosk with your custom Wherewolf app installed.

What should be included in a COVID-19 questionnaire?

While your own questionnaire should be tailored according to your location, industry and local regulations, here are the essentials of what you might include.

  • First name 
  • Last Name
  • Phone number
  • Email address
  • In the past 24 hours have you experienced:
    Fever
    Cough
    Sneezing
    Chills
    New loss of taste or smell
    Head or muscle aches
    Runny or stuffy nose
    Sore throat
    Shortness of breath
  • Have you recently been in close contact with anyone who has exhibited these symptoms? 
  • Have you been in close contact with anyone who has been in quarantine or diagnosed with COVID-19 within the past 14 days? 
  • Have you recently travelled from an area with community spread of COVID-19?

If a guest answers yes to any of the risk factors, you’ll be able to spot these responses in your guest manifest on the day and respond appropriately. It can be worthwhile having a refund or rebooking policy in place, so guests feel they can be honest with their responses without the fear of losing their booking.
