4 Key Questions to Ask When Creating COVID-19 Digital Waivers and Health Screening Forms

This question can help to identify anyone who has travelled from a higher-risk location, so you can actively manage the situation. 

What is your current temperature? 

This question may be an important health screening question for aged care staff and visitors, for healthcare workers and visitors, fly-in fly-out mine sites and other locations where there are high numbers of people interacting. You could choose to include this question on your check-in screening process and have a staff member alert you to any temperatures of 37.3°C or above, for example.

Have you been vaccinated against COVID-19, and if so, when? 

This can be an important question if you’re conducting health screening before providing other vaccinations or healthcare procedures, as there may be recommended waiting times between the two. By asking for a vaccination date, you can delay or arrange a procedure appropriately.
